Please bring dental cards to your examination appointment. Upon request, we will submit a claim to predetermine your insurance benefits; however, this is not required by most plans. Our treatment recommendations are based on your individual needs and not predicated on insurance benefits. However, we will be your partner in helping you to maximize the benefits assured to you by your insurance carrier.
